Terms of Service

These Terms of Service ("Terms") govern access to and use of the StrikerRx Group Order Portal (the "Portal"), operated by Striker Pharmacy and powered by LumifyMD. By accessing the Portal you agree to these Terms on behalf of yourself and the clinic, practice, or healthcare entity you represent.
1. Eligibility & Provider Attestation
The Portal is intended solely for use by U.S.-licensed healthcare providers and their authorized clinical staff for the purpose of submitting valid prescription orders to StrikerRx. By creating an account you represent and warrant that you are: (a) a licensed healthcare provider in good standing or an authorized agent of one; (b) acting within the scope of your license and applicable state and federal law; and (c) authorized to bind your clinic or practice to these Terms.
2. Permitted Use
Orders submitted through the Portal must be for individually identified patients pursuant to a valid prescriber-patient relationship and a lawful prescription. Office-use ordering, resale, or distribution of compounded medications is prohibited except where expressly permitted by federal and state law and confirmed in writing by StrikerRx.
3. Patient List Requirement
Each group order must be supported by a complete, accurate patient list including patient name, date of birth, and the medication, strength, and quantity prescribed for each patient. Patient lists must be provided no later than fourteen (14) calendar days from the date the order is submitted. Failure to provide a complete and accurate patient list within this period may result in cancellation of the order and/or suspension of account privileges.
4. Account Security
You are responsible for maintaining the confidentiality of your credentials and for all activity under your account. Notify us immediately of any suspected unauthorized access.
5. No Medical Advice
The Portal is a workflow tool. It does not provide medical advice, and nothing in the Portal substitutes for the independent professional judgment of the prescribing clinician.
6. Intellectual Property
All Portal software, designs, trademarks, and content are owned by StrikerRx, LumifyMD, or their licensors and are protected by applicable laws.
7. Disclaimers & Limitation of Liability
The Portal is provided "as is" without warranties of any kind. To the maximum extent permitted by law, StrikerRx and LumifyMD disclaim all implied warranties and shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of your use of the Portal.
8. Termination
We may suspend or terminate access at any time for any violation of these Terms, suspected misuse, or as required by law.
9. Governing Law
These Terms are governed by the laws of the State in which Striker Pharmacy is licensed, without regard to conflict-of-law principles.
10. Changes
We may update these Terms from time to time. Material changes will be communicated through the Portal. Continued use after changes constitutes acceptance.
